We want to help make your first appointment as smooth as possible so please read through the information below to know what you can anticipate for your first visit, our office policies and other information that we think you’ll need to know. How do I make sure that my appointment time is confirmed?
Our online booking system requires a credit card on file to schedule appointments. If you book through our office we will ask for personal credit card details to hold your first appointment. Once we have a personal card on file, your appointments will be secured. The card will only be charged if you miss a visit or fail to provide >24 hours notice when rescheduling.
How can I contact the office?
We offer several convenient ways to reach us:
Phone Call us at 650-226-8348 during office hours. Please note that our main office phone number is not a text-enabled line.
Email Reach us at info@perfectlyalignedchiropractic.com. Email is the best way to send attachments such as forms or documents.
Text We offer HIPAA-compliant text messaging for active patients in our system. To send a text, use 415-480-5155. Please be aware that our text system is for existing active patients only — we are unable to accept texts from inactive patients or non-patients. It does not accept attachments; please email those instead.
What payment methods are accepted?
On the day of each appointment we encourage the use of alternate payment methods, to avoid credit card processing fees. We offer a Discount for self-pay patients who pay for services with Cash, Zelle or Check. Credit card processing fees are a significant expense for the business. We hope to minimize the expenditure by encouraging patients to make a habit of choosing cost-free payment methods in our office.
If you have an FSA (Flexible Spending) or HSA (Health Savings) account and wish to enjoy the self-pay cash discount rates, we can provide a superbill (detailed receipt) that you can submit to your benefits administrator for reimbursement.
FSA and HSA cards are for qualified health care expenses. These pre-tax benefits cards can be used to pay for all services in our office ~ chiropractic care, acupuncture, microneedling and massage therapy. A backup personal card is also required to be on file, in the event that your FSA or HSA funds are exhausted.
Your personal card on file will only be charged if you miss a visit or fail to provide >24 hours notice when cancelling or rescheduling.
Do you have a cancellation policy?
Yes. We have a strict policy about late cancellations, late changes and missed visits. To avoid charges, please reach out at least 24 hours in advance to inform us of any appointment changes. The earlier the better. This allows us time to try to fill your appointment, as we often have a wait list of patients who are hoping to get on the schedule for a treatment visit or a massage.
How do I cancel or reschedule an appointment?
1) Use our zHealth Wellness mobile app where you can view all of your scheduled appointments. You can cancel and rebook in the mobile app.
2) If you saved the original confirmation email (sent when the appointment was booked), click on the hyperlink to cancel that specific visit.
3) Contact our front desk by phone (650)226-8348, email info@perfectlyalingnedchiropractic or text our patient-only text line at (415)480-5155. Please leave a detailed message if you do not reach a staff member right away.
Will I receive treatment at my first appointment?
Yes, New Patient Visits for chiropractic/ART® with Dr. Scott Nissenbaum, DC and acupuncture/FSM with Tom Duncan, L.Ac. both include treatment time.
How much will my visits cost?
Please contact our front desk for current self-pay rates for all services. Also, ask about the discount for payments made by cash, check or Zelle.
The New Patient Visit rates for chiropractic, ART® , acupuncture and FSM are the same for everyone - those using insurance benefits and self-pay patients.
Patients who use out-of-network insurance benefits for chiropractic & ART® care with Dr. Scott Nissenbaum will see invoices with CPT codes with “usual and customary rates” per industry standards. You may first have to meet an out-of-network deductible before co-insurance payments begin. Submit a copy of your insurance card to the front desk and we will look up your out-of-network benefits.
What length massage is recommended?
We suggest a 90 minute appointment for first time clients so the therapist can provide a comprehensive, full body treatment & assessment. If you are interested in extending an existing 60 min. massage to 90 minutes, please let us know.
Are there any forms I need to fill out before my visit?
New Patients should receive an email confirmation of the booking which contains a link to our Intake Forms. Please complete & submit these forms online before your visit. If you didn't receive the link, please check your junk email. System emails are sent from NoReply@zhealthehr.com. We're happy to re-send a link to the forms, if needed.
Make sure to familiarize yourself with the Financial Policy while completing the Intake Forms. Remember we have a strict policy about late cancellations and missed visits. To avoid charges, please reach out at least 24 hours in advance to inform us of any appointment changes. Thank you.
What should I wear to my appointment?
You don’t have to wear any special clothing, but it’s important that you are comfortable.
Wear loose, flexible clothing.
Wear your hair up. If you have long hair, it is easier to work with hair off of your neck. We have hair ties, in case you forget.
Wear pants or shorts instead of skirts or dresses.
Avoid heavy, bulky items such as sweatshirts and thick sweaters. Instead wear something thin and stretchy, like activewear, a t-shirt, tank top or a light sweater.
You may bring clothing to change at the office before your appointment.
The more comfortable you are, the more freely you can move during your adjustment.
Where can I park?
There is a small lot between our building (1100 Laurel) and 1140 Laurel. Parking spots along the perimeter are open to any visitor. There are 2 dedicated spots in the center, near the sidewalk, labeled "Perfectly Aligned Chiropractic" which you are welcome to park in, if they are available. Street parking on Laurel is 2 hours. The surrounding streets Brittan Avenue and Walnut Street, are not metered.
Do you take my insurance?
If you wish to use insurance benefits for future treatment visits that include chiropractic adjustments, please let us know.
We are an out-of network provider for selected PPO plans from AETNA, ANTHEM BLUE CROSS, BLUE SHIELD, CIGNA, UNITED HEALTHCARE and UMR. You may have to meet an out-of-network deductible before benefits apply. We can check your benefits if you provide a copy of your insurance card, front & back. We'll let you know if your policy covers chiropractic care in our office.
You will be asked to review & sign a document entitled "Information for Insured Patients."
We are not contracted with and therefore do not accept: HMOs, EPOs, Kaiser, Medicare or Workers’ Comp.
Do you accept FSA or HSA cards?
Yes, if you have a pre-tax health benefits card (FSA or HSA), you may use it to pay for any services in our office ~ chiropractic, ARTⓇ, acupuncture, microneedling, FSM, and massage. If you pay with a personal credit card and want to seek reimbursement from your FSA or HSA provider, ask for a “superbill,” which is a detailed receipt that is commonly accepted for this purpose.
Do you offer a cash discount?
Yes! We offer a cash discount on treatment visits for self-pay patients when payment is made by cash, check, or Zelle. Please note that this discount does not apply to the initial new patient visit, nor to patients who use insurance. Our front desk can provide you with the current discount rate.
How do I make a Zelle payment?
If you are registered with Zelle through your bank, you may send fee-free payments to Perfectly Aligned Chiropractic via Zelle. Ask the front desk for details so you can find our Zelle account.
Do you accept Medicare or Kaiser?
No, we are not contracted with Medicare or Kaiser.
Do you have a newsletter?
Yes, in fact, when new patients join our practice we invite them to join our mailing list to receive Good Vibes, our newsletter. If you wish to opt out and discontinue reading our monthly publication, which includes insights from Dr. Scott, memos from the back office staff, holiday schedules, and more - please click the “unsubscribe” button at the bottom of the newsletter.

What are your COVID policies?
In San Mateo County, all restrictions have been lifted. If wearing a mask will make you more comfortable in our office you are welcome to do so. Please let us know if you would also like us to wear our masks when you visit. We also have masks available if needed.
You're also welcome to wait in the courtyard if you don’t feel comfortable in our lobby ~ there's a bench and a chair in front of our suite.

Do you have a sick policy?
We simply ask that if you are actively sick, may be contagious, or are living with someone who is sick and who may be contagious, please contact us ASAP to reschedule your visit.

Look for Suite D in the corner of the courtyard, between Bay Area Foot Care and APT Fitness. We share space with Dr. Jessica Snowden Patel; both names are on the front door.
