We want to help make your first appointment as smooth as possible so please read through the information below to know what you can anticipate for your first visit, our office policies and other information that we think you’ll need to know. How do I make sure that my appointment time is confirmed?

Our online booking system now requires a credit card on file to schedule appointments. If you book through our office we will ask for personal credit card details to hold your first appointment. Once we have a personal card on file, your appointments will be secured.

On the day of each appointment we encourage the use of alternate payment methods, to avoid credit card processing fees. We accept Zelle (which is a free & secure payment transfer system offered by most banks), cash and checks.

We also accept FSA and HSA cards, designated for qualified health care expenses. These pre-tax benefits cards can be used to pay for chiropractic care and massage therapy in our office. A backup personal card is also required to be on file, in the event that your FSA or HSA funds are exhausted.

The card will only be charged if you miss a visit or fail to provide >24 hours notice when rescheduling.

Do you have a cancellation policy?

Yes. We have a strict policy about late cancellations, late changes and missed visits.  To avoid charges, please reach out at least 24 hours in advance to inform us of any appointment changes.  The earlier the better. This allows us time to try to fill your appointment, as we often have a wait list of patients who are hoping to get on the schedule for a treatment visit or a massage.

Will I receive treatment at my first appointment and how much will it cost?

The initial appointment with Dr. Scott is a premium service, one hour in length, which includes an exam, review of your health history, treatment plan & the initial treatment. The cost of this visit is $199 which can be paid for with Zelle, cash, check, or a credit card. If you have a pre-tax health benefits card (FSA or HSA), we encourage you to use it to pay for services in our office ~ chiropractic care & massage therapy. 

Follow-up treatment visits (25 minutes) cost $110.

What length massage is recommended?

We suggest a 90 minute appointment for first time clients so the therapist can provide a comprehensive, full body treatment & assessment. If you are interested in extending an existing 60 min. massage to 90 minutes, please let us know.  

Are there any forms I need to fill out before my visit?

You should have received an email confirmation of the booking which contains a link to our Intake Forms.  Please complete & submit these forms before your visit.  If you didn't receive the link, please check your junk email.  We're happy to re-send the forms, if needed.

Make sure to familiarize yourself with the Financial Policy while completing the Intake Forms.  Remember we have a strict policy about late cancellations and missed visits.  To avoid charges, please reach out at least 24 hours in advance to inform us of any appointment changes.  Thank you.

What should I wear to my appointment?

You don’t have to wear any special clothing, but it’s important that you are comfortable.  

  1. Wear loose, movable clothing.

  2. Wear your hair up. If you have long hair, it is easier to work with hair off of your neck. We have hair ties, in case you forget.

  3. Wear pants or shorts instead of skirts or dresses.

  4. Avoid heavy, bulky items such as sweatshirts and thick sweaters. Instead wear something thin and stretchy, like activewear, a t-shirt, tank top or a light sweater.

You may bring clothing to change at the office before your appointment.

The more comfortable you are, the more freely you can move during your adjustment.

Do you take my insurance?

If you wish to use insurance benefits for future treatment visits that include chiropractic adjustments, please let us know. 

We are an out-of network provider for AETNA, ANTHEM BLUE CROSS, BLUE SHIELD, CIGNA, UNITED HEALTHCARE and UMR. You may have to meet an out-of-network deductible before benefits apply. We can check your benefits if you provide a copy of your insurance card, front & back. We'll let you know if your policy covers chiropractic care in our office.  

You will be asked to review & sign a document entitled "Information for Insured Patients."

We are not contracted with and therefore do not accept: Kaiser, Medicare or Workers’ Comp insurance.

Do you accept FSA or HSA cards?

Yes, if you have a pre-tax health benefits card (FSA or HSA), you may use it to pay for your treatments ~ chiropractic & massage.  If you pay with a personal credit card and want to seek reimbursement from your FSA or HSA provider, ask for a “superbill,” which is a detailed receipt that is commonly accepted for this purpose.

Do you accept Zelle payments?

Yes! If you are registered with Zelle through your bank, you may send fee-free payments to Perfectly Aligned Chiropractic via Zelle.

Do you accept Medicare or Kaiser?

No, we are not contracted with Medicare or Kaiser.

Do you have a newsletter?

Yes, in fact, when new patients join our practice we invite them to join our mailing list to receive Good Vibes, our newsletter. If you wish to opt out and discontinue reading our monthly publication, which includes insights from Dr. Scott, memos from the back office staff, holiday schedules, and more - please click the “unsubscribe” button at the bottom of the newsletter.

Where can I park?

There is a small lot between our building (1100 Laurel) and 1140 Laurel.  Parking spots along the perimeter are open to any visitor. There are 2 dedicated spots in the center, near the sidewalk, labeled "Perfectly Aligned Chiropractic" which you are welcome to park in, if they are available. Street parking on Laurel is 2 hours.  The surrounding streets Brittan Avenue and Walnut Street, are not metered.

What are your COVID policies?

San Mateo County restrictions have been lifted. If wearing a mask will make you more comfortable in our office you are welcome to do so. Please let us know if you would also like us to wear our masks when you visit. We also have masks available if needed.

You're also welcome to wait in the courtyard if you don’t feel comfortable in our lobby ~ there's a bench and a chair in front of our suite.

Do you have a sick policy?

We simply ask that if you are actively sick, may be contagious, or are living with someone is sick and who may be contagious, please contact us ASAP to reschedule your visit.

Our single-story building is at the SW corner of BRITTAN and LAUREL Streets, one block west of El Camino Real.

Look for Suite D in the corner of the courtyard, between Bay Area Foot Care and APT Fitness.  We share space with Dr. Jessica Snowden Patel; both names are on the front door.
